Yes and No

It can be painful if you want it to be, or as gentle as the feeling of butterfly wings caressing your skin. It all depends on how you play, you can crank up the power until it hurts like hell and you cramp up, and that is ideal for BDSM sessions.

Or, you can lay back and have it deliver incredibly sensual sensations that can't be recreated any other way. I prefer the latter and regularly orgasm from stimming. I occasionally like the prickly feeling of an electro pinwheel on my buttocks or even perineum but my favourites are more like streams of energy, pulses, or having my penis massaged by someone with rapidly vibrating rollers for fingers. Pinwheels can be so much fun, especially if using them on others. I know what you mean about settings, once you get into that zone just enjoy it as too much tweaking can definitely bring it all crashing down.

My ElectraStim Flick with their Prestige silicone adjustable rings will do it on some of the wave like patterns but I also get it from the Med-Fit TENS machine with just a pad on the frenulum area and another on the base of the balls. The Modulated TENS is actually described in the manual as Pain relief with a massage effect and it says 80Hz with modulated 70-180μs pulse width. I don’t know if your gear will let you modulate manually by turning the knobs while it’s running but that might let you create more patterns.
